Re: 2010 Results

Dear Dan;

As 2010 is coming to a close, I would like to personally thank you for a tremendous year. You have greatly contributed to ESC from the medical arena. As you know, under the fringe benefit category, our medical costs have always been the largest dollars that are spent during a calendar year. This year has been quite the success story for ESC with the increased management you have given to the On Site clinic, we had a major decrease in medical over our budgeted numbers. I would like to share the positives in this letter and are willing to have you share this as any type of reference or testimonial from both the cost savings measures along with the increase in positive employee morale.

This year our medical cost have decreased over $450,000 after deducting the full cost of the medical services you provide on-site. In addition, our workers compensation has decreased by $52,000 with the prompt medical services you provide along with the assistance of job assessments.

You are highly regarded and respected for the knowledge in the medical field in which you serve. We respect your knowledge of the latest trends and developments and as well as the prediction in the field of industrial medicine. Your strong personal commitment and your high ethical practices are greatly known by all.

I would be glad to share any additional information regarding the benefits of having you serve in the capacity of Clinics on Site here at ESC.

Sincerely,
Jan S. Comer

In the spring of 2006, Cardinal FG entered into a partnership with Dan Whisnant (and Clinics On-Site, PLLC) to provide medical services for our employees on a set schedule each week.

Without a doubt, it has been one of the best services we have offered our employees. They love the convenience of being able to see the FNP if they are at work and many come in when they are not scheduled to work in order to have their medical care followed here. We are able to have workers comp injuries follow up with the in-house clinic at many times saving money, time for the employee, and assuring the employee and Cardinal of quality care following an on-the-job injury. We are also able to do our pre-employment/post-offer physicals on-site, thus assuring a consistency in evaluations and follow-ups as needed.

The thoughtfulness and quality of care provided by Dan and his employees are top-notch. Our employees love the convenience of health care on-site and, realistically, we believe some employees have benefited from this care who otherwise would not have sought medical treatment.

We look forward to continuing our partnership with Clinics On-Site and certainly would recommend them to any company considering providing in-house medical care for their employees.

Respectfully,
Tina Robertson

Dan Whisnant has provided Nurse Practitioner services to NGK Ceramics USA, Inc. since October of 2006. During this time Dan has done an outstanding job with employee screenings, evaluations, and management of medical conditions including workers' compensation claims.

Dan was our first FNP and came to us following high recommendations from neighboring employers. We have not been disappointed! I have never heard a negative comment from an employee regarding Dan or the services that he provides for NGK. Dan's professionalism and reputation are impeccable! We only hope that Dan will continue to assist us as we place more emphasis on wellness in our workplace.

Sincerely,
Kristy H. Hall, PHR

It gives me great pleasure to provide Dan Whisnant; Clinics On-Site with a reference.

I made my initial acquaintance with Mr. Whisnant in 2004 when we met when I first started working for a company where Mr. Whisnant had started a clinic. It was my first exposure to an on site clinic and I was very impressed with how well he operated the clinic, his professional mannerism, the positive effect his clinic brought to the plant and the cost savings shared by the company and employees. When I left that employer and began working for PGI I asked him to consider opening a clinic at our company on his off days. He has operated the on site clinic at the Mooresville, NC manufacturing facility for the past 6 years. During this time he has also treated employees from our corporate headquarters located in Charlotte.

Mr. Whisnant is in possession of some of the best qualities of a family nurse practitioner. The four words which come to mind to describe him are: Selflessness, Benevolence, Diligence and Congeniality. Guided by those principles, Mr. Whisnant worked diligently, responsibly and considerately towards his patients. Professionally and ethically, Mr. Whisnant has proven himself to be a highly qualified FNP and his Clinics On-Site business continues to expand throughout the community.

In November the decision to offer onsite clinics at all our manufacturing facilities in the US was approved and a vendor was selected who could supply clinics to all our locations. The Clinics On-Site which Mr. Whisnant will close on December 31st.

Should you have any questions concerning Mr. Whisnant and his Clinics On-Site, please do not hesitate to contact me.

Sincerely,
